Everyone’s situation is unique and each individual’s care plan will reflect his or her specific needs. These are some examples of what a service package may include:
Case Management:
Physical and mental health assessments; Personalized care plans; Information and referrals; Home visits; Ongoing telephone support, Assistance with benefits and entitlements, Ongoing medical monitoring by a geriatric nurse practitioner.
Medical Escorting:
Staff will be provided to attend medical appointments with older adults.
In-Home Support:
Housekeeping; Laundry; Light shopping.
Transportation:
Riverdale area: free wheelchair-accessible vehicle to/from the Riverdale Y; Washington Heights/Inwood area: free wheelchair-accessible vehicle to/from the Washington Heights Y; Washington Heights/Inwood area: $2 taxi rides to doctor appointments and for other local errands.
Kosher Meals:
Daily meals at the Washington Heights and Riverdale Y Senior Centers, access to a kosher food pantry, holiday meals for pick up and/or delivery.
